Transportation
Waiver Form

Student are REQUIRED to take the designated bus transportation to all off-campus events. However, completing this form and returning it to Dr. Howard Stephens allows the student's PARENTS (and only their parents) to provide their own transportation to/from an event for their own child(ren). Students are NOT allowed to drive themselves to any event.
